I think he just manifested his hallucination, because when the nurse walks in and his throat is just opening(awesome effect btw). I hope Nina stops being a cocky bitch. She seems to deflect everything in the oddest way possible. It looks like eventually Olivia will take her job offer and use it to spy on massive and learn more of the pattern.
